Care Partners Program

The Care Partner Program creates a partnership among patients, loved ones, and staff to promote healing and recovery Patients can choose a Care Partner to participate in their care while they are in the Hospital The Care Partners may:

• Provide patients with emotional and spiritual support
• Learn about relapse prevention and coping strategies
• Join selected groups and activities
• Respects families' expertise and experience
• Empowers care partners and patients by providing education and support
• Help plan for discharge and provide continuity from hospital to home
